The Community Intervention Program services work with you to improve skills and restore functioning, that has been affected by mental illness. We will design a plan to stabilize your symptoms, help you learn about your illness, and avoid hospitalizations. Services can be provided in your home, in the community (at other agencies), or in our clinic. We work alongside other LRCMHC providers, including doctors and nurses.
Referrals: Referrals are made by your psychiatrist (or other professional staff). You first meet with a mental health professional and review the treatment plan, assessing your needs and determining if you could benefit from our services.
For more information about ANY of our services, please contact firstname.lastname@example.org.
